Health Coverage: Three Plans: Keystone HMO, Keystone POS and Independence blue Cross Choice. Employee contributes according to coverage fee schedule. Effective first day of month following 90 days of continuous employment. If you do not select medical coverage at that time you will not be eligible until open enrollment, Nov. 1st.
Section 125/Flex 1: Regarding health coverage contribution. Tax deferral: not subject to FWT, State(PA), and Social Security tax in your paycheck: extra dollars in net pay. Optional and effective with health coverage date.
Dental Coverage: Horizon Healthcare Dental Program: employee contributes 100% according to coverage fee schedule. Effective first day of month following 90 days of continuous employment.
Life Insurance: Effective first day of month following 90 days of continuous employment. $5,000 up to 5 years, $7,500 5 to 10 years, $10,000 10 to 15 years, $15,000 after 15 years of service. F&H contributes 100%. Coverage is discontinued upon date of termination from Finnaren & Haley.
Accidental Death & Dismemberment: Death due to Accident or Loss of Limb: same benefit as Life Insurance. F&H contributes 100%. Effective first day of month following 90 days of employment, coverage is discontinued upon date of termination.
Short Term Disability: Non-work related injury or illness. 70% of weekly wage exclusive of overtime to a max of $300 weekly. Payment period is 26 weeks. Employee contributes $.75 a week. Effective first day of month following 90 days of continuous employment.
Workers Compensation: Work related injury or illness; 66 2/3% of weekly wage based on gross earnings. F&H pays 100%. No waiting period. Zurich is insurance carrier. Must use Zurich network providers for medical treatment to be compensated.
401K Plan: Tax Deferred Plan. Open enrollment April 1 and October 1. Contribute 1% to 18%. Age 19. DE and NJ exempt from FWT and state tax; PA exempt from FWT. 100% vested after three years of employment and 1000 annual work hours.
Vacation: See attached schedule.
Personal Hours: Effective after 90 days Training Period. Refer to memo on store bulletin board. Hours not used paid as bonus.
Holidays: 8 (New Years Day, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas). Store Division: company designates 2 remaining days, Plant: Good Friday & Day After Thanksgiving. Effective immediately.
Credit Union: Savings through payroll deduction. Employee elects amount. No waiting period.
GlobalFit: Fitness program. Discounted through F&H. No waiting period.
F&H Discount: F&H Paint 40%; Sundry Items 30%. Waiting period 90 days.
Discount Coupons: Immediate F&H discount to various amusement facilities and BJs Wholesale Club.
Car Insurance: Travelers. Employee contributes 100%. PA only.
